Overview
The Clinical History form is divided into two main sections:- Heredo-familial history (Antecedentes Heredo-Familiares)
- Pathological history (Antecedentes Patológicos)
A patient must be selected before you can access and edit the Clinical History form.
Section 1: Heredo-familial history
This section documents the health status and conditions of the patient’s family members to identify hereditary risk factors.Family members tracked
The form includes predefined rows for:- Madre (Mother)
- Abuela Materna (Maternal Grandmother)
- Abuelo Materno (Maternal Grandfather)
- Padre (Father)
- Abuela Paterna (Paternal Grandmother)
- Abuelo Paterno (Paternal Grandfather)
- Hermanos (Siblings)
You can add additional relatives beyond the standard family tree using the “Mostrar Más Parientes” option.

Section 2: Pathological history
This section documents the patient’s own medical history, including chronic diseases, surgeries, hospitalizations, and more.Chronic diseases (Enfermedades Crónicas)
Select from common chronic conditions:- Diabetes Tipo 1, Diabetes Tipo 2
- Hipertensión Arterial
- Asma, EPOC
- Epilepsia
- Cardiopatías, Artritis Reumatoide
- Osteoartritis, Osteoporosis
- Hipotiroidismo, Hipertiroidismo
- Enfermedad Renal Crónica
- Hepatitis B, Hepatitis C
- VIH/SIDA
Previous surgeries (Cirugías Previas)
For each surgery, record:- Type: Name of the surgical procedure (e.g., “Apendicectomía”, “Cesárea”)
- Date: When the surgery was performed
- Notes/Complications: Any relevant details or post-operative complications
Click the “Agregar Cirugía” button to add multiple surgical history entries.
Hospitalizations (Hospitalizaciones)
Document past hospital admissions:- Reason: Why the patient was hospitalized
- Date: Date of admission
- Duration: Length of hospital stay
- Notes: Additional relevant information

Visual organization
The form uses color-coding for family lineages:- Pink background (#ffe6e6): Maternal line (Madre, Abuela Materna, Abuelo Materno)
- Blue background (#e6f3ff): Paternal line (Padre, Abuela Paterna, Abuelo Paterno)
- Orange background (#fff3e6): Siblings (Hermanos)

Click the “Guardar Antecedentes” button at the bottom of each section to save the entered information.Best practices
Collecting family history
- Interview the patient about known family medical conditions
- Focus on first-degree relatives (parents, siblings) first
- Document age at diagnosis for hereditary conditions
- Note any patterns of disease across generations
Recording pathological history
- Be thorough but concise
- Use standard medical terminology
- Include dates whenever possible
- Update regularly as new information becomes available
